• I just use the H and P
  That is pretty much all I used it for ... can get photocopies and even better forms are out there.
  

• Get Hancock instead
  Try "The Practitioner's Pocket Pal, Ultra Rapid Medical Reference" 2nd ed. by Jim Hancock (about $17).. Maxwell's is very cute but very small.

• NOT MUCH HELP!!
  THIS WAS NOT AT ALL WHAT I EXPECTED. LIMITED INFO. VERY, VERY SMALL PRINT. I WOULD NOT BUY ANOTHER. FLIMSY.

  very good reference for clinics, easy, quick look up accessible info that fits in your coat pocket
